For most patients, care begins—and stays centered—in one of our Welcome Health clinics. This is where we’re able to provide the most complete and coordinated care, with access to your full care team, on-site services, and the tools needed to support both preventive care and ongoing health needs.
Clinic visits allow us to spend time getting to know you, understanding your health history, and addressing concerns before they become more complicated. From routine checkups to managing chronic conditions, the clinic is where we can offer the most comprehensive support.
What We Provide in Our Clinics
In our clinics, patients receive a wide range of primary care services designed to support long-term health and day-to-day wellbeing. This includes annual wellness visits, preventive screenings, and vaccines, as well as same-week sick visits when something comes up unexpectedly.
We also focus on chronic condition management, medication reviews and refills, mobility and fall-prevention support, and coordination with specialists, labs, and diagnostic services. Having these services connected through one care team helps reduce confusion and ensures nothing falls through the cracks.

Virtual Visits for Established Patients
For patients already receiving care with Welcome Health, virtual visits provide a convenient way to stay connected between clinic visits. These visits are well suited for follow-ups, medication questions, reviewing test results, or checking in on ongoing concerns.
Virtual care is designed to complement—not replace—clinic-based care. By keeping the clinic at the center of your care plan, we ensure that virtual visits remain well coordinated and grounded in a complete understanding of your health.
Care in the Home, When Appropriate
In some situations, an in-home visit may be the safest or most practical option. These visits are scheduled in advance, limited in scope, and offered only when a clinic visit isn’t feasible and when care can be safely provided at home.
Availability depends on clinical needs, location, team capacity, and whether the visit is appropriate for the situation. Home visits are not urgent or on-demand, but they can provide meaningful support during certain transitions or temporary limitations.
Starting Care With Welcome Health
Most new patients begin their care journey in the clinic. This allows us to complete a thorough evaluation, establish care plans, and build a strong foundation for ongoing support.
In limited cases, starting care with an in-home visit may be appropriate. If you believe you or a loved one may need to begin care at home, we encourage you to call us. Our team will take the time to understand your situation and determine whether in-home care is an option in your area.
Support for Established Patients
For patients already receiving care with us, in-home visits may be available when medically necessary and when traveling to the clinic isn’t possible. These visits are planned ahead of time and focused on short-term support, such as after a hospital or rehab stay, during temporary mobility limitations, or when reviewing new medications or care routines.
They are meant to help you stay safe and supported until you can return to clinic-based care.
